Observed small gap at the bottom of the door to the outside of the facility. Door is only opened for deliveries and is maintained close all other times. [CA] Eliminate gaps at the bottom of the door to prevent the entrance of vermin.

[COS] Cockroaches were killed and removed immediately by maintenance. A follow up inspection will be conducted within 24 hours to verify that facility is being maintained free of vermin.
