Facility serves refried PHFs and no employee observed to have food safety certificate or handlers card. [CA] Facility that prepares and serves open food shall have employees with food handler cards and one employee with food safety certificate.
A required permit, food safety certificate, trained manager, or posted sign was missing or not current. These show the business is following the rules and has trained staff.

Reach in cooler by front door measured to have: -Chutney at 44F -Boxed samosa at 43F -Ambient temperature at 45F [CA] Maintain PHFs at 41F and below for cold holding.
Food was not kept cold enough, hot enough, cooled fast enough, or cooked or reheated to the right temperature. Bacteria grow quickly in the "danger zone" between 41°F and 135°F, which makes this one of the most important food-safety checks.

Rodent droppings observed scattered under shelves along walls of facility and in back equipment area. Rodent droppings also observed under the end caps of aisles. One dead adult cockroach observed in trap behind screen where chairs and tables are stored. [CA]Facility shall clean and sanitize all droppings and dead cockroaches from affected areas. Ensure traps changed regularly. Follow up shall be conducted to ensure droppings have been cleaned. Facility shall provide last pest control report and contract with pest control during follow up inspection.

No rodents, insects, birds, or animals
Observed rodent droppings around food containers on pallets observed rodent droppings on the floors by shelves. Observed two dead rodents on traps. Observed dead cockroahces on glue traps. [CA] The premises of each food facility shall be kept free of vermin. A food facility shall not operate when there is a vermin infestation that has resulted in the contamination of food contact surfaces, food packaging, utensils, food equipment, or adulteration of food(s). The food facility shall cease operation of the food facility immediately. The food facility shall remain closed until: there is no longer evidence of a vermin infestation; all contaminated surfaces have been cleaned and sanitized; and contributing factors such as cleaning, repairs, and the elimination of harborages have been resolved.
