Major: Measured 0ppm quat concentration at warewash sink. Observed active warewashing. Observed mounted quat dispenser not working properly at warewash station. [CA] Manual sanitization shall be accomplished in the final sanitizing rinse by contacting a chlorine solution of 100 PPM concentration for at least 30 seconds or by contacting a quaternary ammonium solution of 200 PPM concentration for at least 60 seconds. [COS] Operator directed to re-sanitize all equipment/utensil previously stored in third compartment sink. Operator directly poured quat solution into third compartment sink. Measured final quat concentration at 400ppm. Minor: Found red like substance on white interior panel of ice machine. [CA] Routinely clean.
