Some drain flies were observed in the preparation and some in the storage areas. [CA] Eliminate all insects and rodents from the facility by a legal method. Please schedule a pest control company intervention immediately, and conduct an intervention and a thorough cleaning. Report of the pest control company report must be emailed to the district inspector within 72 hours.
The inspector found mice, rats, cockroaches, flies, or evidence of them (like droppings) inside the business, or conditions that could attract them. Pests can spread bacteria to food and surfaces, so this is treated as one of the more serious findings.

Couple of wet/soiled wiping cloths were stored on the counters. [CA] Wet/soiled wiping cloths must be stored in a bucket that has standard sanitizer concentration in between use which is 200 ppm quaternary ammonia or 100 ppm chlorine (bleach).
Dishes, equipment, or surfaces that touch food were not properly washed, rinsed, and sanitized, or sanitizer was not at the right strength. Clean surfaces help stop germs from spreading to food.
