A facility cannot operate if there is a vermin infestation that leads to contamination of food contact surfaces, packaging, utensils, food equipment, or adulteration of food. Thoroughly clean and sanitize areas of dead cockroaches. Monitor glue traps daily and replace as needed. [COS] Employee discarded glue traps. Note: Facility receives monthly pest control services. Reviewed pest control report dated 11/14/25, no activity reported.
