Observed knives placed out for customer self-service are not protected from contamination. [corrective action] Utensils placed out for customer self-service shall be stored in a manner where it is protected from contamination, such as storing only with the handles of the utensils exposed.

Observed dead cockroaches in a monitoring trap next to the walk-n freezer. No live cockroaches were observed at this time. Per owner, the trap was left there for a long time and forgot to remove. [corrective action] Remove the dead cockroaches. Ensure there are no cockroaches in this facility. Recommend setting another monitoring trap to observe for any new cockroach activity. If cockroaches are observed, contact pest control company immediately. Note: This facility currently does not have pest control service. It is strongly recommended to have routine pest control service to prevent vermin infestation.
