[CA] Whenever sanitizing utensils/equipment is conducted using a three compartment warewash sink, there must be a 100-ppm residual chlorine or 200 ppm quaternary ammonia sanitizer level on the sanitizing compartment. The person in charge refilled one sink of the warewash sink with lukewarm water and added bleach manually to create a 100-ppm chlorine solution to sanitize the utensils (COS).

Please conduct a regular cleaning and replace the traps immediately.

Observed 1 live cockroach tried to crawl out from floor sink, 1 live cockroach and 5+ dead cockroaches in the floor sink near serving table. 3 dead cockroaches observed on the floor in the back of the reach-in freezer. 1 dead cockroach observed on the floor near 2 compartment warewash sink. [CA] The premises of each food facility shall be kept free of vermin. A food facility shall not operate when there is a vermin infestation that has resulted in the contamination of food contact surfaces, food packaging, utensils, food equipment, or adulteration of food(s). Food facility can only serve pre-packaged items until: there is no longer evidence of a vermin infestation; all contaminated surfaces have been cleaned and sanitized; and contributing factors such as cleaning, repairs, and the elimination of harborages have been resolved. [SA] Facility janitor vacuum the cockroaches in the floor sink. Cleaned the floor. Pest control service is schedule by district. Facility is instructed to only serve pre-packaged food item until violation is corrected. Employee rewashed and sanitized utensils located around the serving table area.
