On the counter-top, observed a container of sliced white cheese stored with a TPHC label for two minutes prior, and measured at 69F. Per operator, was not able to determine the time the sliced white cheese was removed from temperature control. [CA] When time only, rather than time and temperature is used as a public health control, PHFs shall be time marked to indicate when item is removed from temperature control, discarded if not consumed or served within 4 hours, and written procedures shall be readily available for review. [COS] Operator discarded container of sliced white cheese.

In the dry storage area, observed liquid accumulation on the floor below shelving. [CA] Ensure flooring is properly repaired and any gaps sealed to prevent possible harborage of vermin. Floor shall be frequently cleaned and maintained to prevent harborage of vermin.
