Adequate ventilation/lighting; designated areas, use
Gas cooker is not under ventilation. [CA] Provide approved exhaust hood ventilation over all cooking equipment as required to effectively remove cooking odors, smoke, steam, grease, heat, and vapors. Observed ovens and two burner stock pot under ventilation hood without 6" overhang on either side. [CA] There shall be a minimum of 6 inches overhang of ventilation hood over cooking equipment. Lights were not working in ventilation hood. [CA] Repair/replace lights. Observed heavy accumulation of grease on hoods in cooking area. [CA] Clean mechanical exhaust hood and/or filters of grease accumulation.
Floors, walls, ceilings, lighting, ventilation, or garbage areas needed repair or cleaning. These are usually less urgent than food-handling problems but still need to be fixed.

Employees keys and purse are stored on shelving with food equipment. [CA] No person shall store clothing or personal effects in any area used for the storage and preparation of food or food contact surfaces.

Observed missing caulking in the coving throughout the facility. [CA] Repair/replace caulking in coving. Observed heavy accumulation of food grease/debris around cooking equipment. Observed pork and fish thawing in standing water at preparation sink. [CA] Frozen potentially hazardous food shall only be thawed in one of the following ways: 1) under refrigeration that maintains the food temperature at 41°F or below, 2) completely submerged under potable running water for a period not to exceed two hours at a water temperature of 70°F or below, and with sufficient water velocity to agitate and flush off loose particles into the sink drain, 3) in a microwave oven if immediately followed by immediate preparation, 4) as part of a cooking process.
Food was not kept cold enough, hot enough, cooled fast enough, or cooked or reheated to the right temperature. Bacteria grow quickly in the "danger zone" between 41°F and 135°F, which makes this one of the most important food-safety checks.

Clams in tanks did not have shellstock tags available. Receipts were provided for grocery store they were purchased from. Noodles in cold holding insert measured 54F. Per PIC, it had been brought out of walk in fridge 40 minutes prior. Rice noodles on counter measured 78F, noodles had been delivered earlier in the morning. No preparation observed. [CA] PHFs shall be held at 41°F or below or at 135°F or above. [COS] Noodles were placed back into the reach in fridge.
