Noted strong odor of smoke in the kitchen. Upon investigation, observed that the oven was in-use to reheat foods, but the ventilation hood had not be turned on. Per Michelle, she had had a meeting earlier today and forgotten to turn the hood on. [CA] Provide approved exhaust hood ventilation over all cooking equipment as required to effectively remove cooking odors, smoke, steam, grease, heat, and vapors. Ensure ventilation hood is turned on while cooking equipment is in-use.

*REPEAT VIOLATION* Two dead, partially decomposed rodents observed on a sticky trap on the floor in the corner of the dry storage room. Photographs taken for documentation purposes. *No other evidence of rodent activity observed* Per staff, facilities handles pest control (via sticky traps) in the interior of the kitchen, and Advanced IPM performs regular pest control treatments around the exterior of the building. *Note: Facility has a past history of rodent activity. Consulted with J. Zertuche and J. Rubingh regarding vermin findings. [CA]: The premises of each food facility must be maintained free of vermin. A facility cannot operate if there is a vermin infestation that leads to contamination of food contact surfaces, packaging, utensils, food equipment, or adulteration of food. Keep doors closed where possible to prevent the intrusion of vermin. Seal any gaps, holes, cracks, or crevices in or around the kitchen area that may allow for the entrance or harborage of vermin. The facility may remain open under the condition that: The facility may not assemble/repackage food, or portion or handle open foods until approval is obtained from this department during the follow-up inspection. A follow-up inspection will be conducted within 48 hours to ensure that the facility is free of rodent activity. [SA] Sticky traps with rodents on top discarded.

Chicken salad measured 46F-49F in the 2-door upright cooler in the kitchen area. Ambient of the unit measured between 43F-46F. Per the PIC, unit ambient had read 41F less than 1.5 hours prior. [CA] Potentially hazardous foods (PHFs) shall be held at or below 41F to prevent the growth of bacteria. Refrigeration must be capable of maintaining PHFs at or below 41F. [SA] Chicken salad to be served or discarded by the end of the lunch period (less than 4 total cumulative hours out of temperature control). Refrigerator technician scheduled.
