[CA] Reheating for hot holding shall be done rapidly, and the time the food is between 41°F and 165°F shall not exceed two hours. Any ready to eat foods such as the broccoli soup may be reheated to >135°F for hot holding. [COS] PIC reheated the broccoli soup inside the microwave. Reheated soup measured to be 166F inside microwave and measured the temperature at 155F in the hot holding unit.
In plain English
Food was not kept cold enough, hot enough, cooled fast enough, or cooked or reheated to the right temperature. Bacteria grow quickly in the "danger zone" between 41°F and 135°F, which makes this one of the most important food-safety checks.
