Facility had a change of ownership in 2022. Health permits are nontransferable and new owner is directed to apply for a change of ownership and pay all applicable fees. [CA] A food facility shall not be open for business without a valid health permit. Facility will be subject to 100% penalty fees. Within 48 hours new owner shall submit the change of ownership food facility evaluation form and pay all applicable fees. Failure to complete process within 48 hours will result in facility being ordered closed for lack of valid health permit.
