    Inspector's note: Based off of the DFPS investigation, the daycare was neglectful by leaving a child unsupervised in a playground for 13 minutes. The child then went through three gates and went to the front of the operation to get back into the center. The caregiver was unaware the child was left outside. Risk level: High.
