Through interviews of staff, the victim, and a collateral adult, and through review of video, it was determined a caregiver was not properly supervising the children in care when one young child left the classroom without their knowledge and exited the building. The young child was returned to the operation by an adult not associated with the operation. The young child was unsupervised by the operation for 5 minutes and 46 seconds. Risk level: High.
