  Inspector's note: On 04/08/2025, one child was left unattended in the Grow Fit classroom for approximately 3-5 minutes. The person in charge, did not notify the parent of the incident immediately, and/or during pick-up the day of the incident. The following day, 04/09/2025, the parent was not notified of the situation until pick-up, at approximately 6-6:30PM, more than 12 hours after the incident occurrence. Risk level: High.   Inspector's note: On 08/08/2024 two infants, 12 months of age was grouped with school-age children, 4- 10 years of age. Based on the documentation observed the infants were present with the mixed group for approximately 10-15 minutes. A total of 18 children were signed in at one designated time. On 08/09/2024, one infant 12 months of age was grouped with school-age children, 4- 10 years of age. Based on the documentation observed the infant was present with the mixed group for approximately 20 minutes. A total of 15 children were signed in at one designated time. Risk level: Medium High.
