Inspector's note: While in care, a child exited through a gate on a playground without the knowledge of the teachers and was unsupervised outside of the playground area for approximately 2 minutes. Risk level: High. Corrected by 2023-10-12. - Critical Parental Communication - Situation that Placed a Child at Risk
Inspector's note: After a child exited the playground area at 1:00 pm on 9/19/2023, the child?s lead teacher did not inform the parents and the administrative staff of the incident. Additional teachers, who were outside at the time of the incident, informed the office staff of the situation around 2:15 pm. The office staff then notified the director around 3:30 pm; the director informed the parents about the incident with their child at 3:30 pm. Risk level: High. Corrected by 2023-10-12.
