Inspector's note: It was determined throughout the course of the HHS Investigation the operation failed to report to CCR within the required time frame. The incident occurred on 1/31/25. The incident was not reported to CCR until 2/3/25. Risk level: High. Inspector's note: It was determined throughout the course of the HHS Investigation the caregiver failed to maintain the level of supervision required for a young child in care while on the playground. Surveillance footage revealed the child was out of the caregiver?s view for a total period of 6 minutes. Within those 6 minutes, the child was left alone outside unsupervised on the enclosed playground for 3 minutes and 15 seconds. Risk level: High. Corrected by 2025-03-03.

Inspector's note: It was determined throughout the course of the HHS Investigation that a caregiver withheld food from a child in care when the child refused to put their hands in their lap.
