Inspector's note: Based upon interviews, staff did not do a name to face after transitioning children back into the classroom from a bathroom break. This resulted in a child being left unsupervised in the hallway near the classroom for approximately 1-2 minutes until the child was found by the director. Risk level: High. Corrected by 2023-12-27.
