  • Non-critical Children's Records - Health Statement
Inspector's note: During a child's records audit, there were 3 of 8 children's records missing a health statement from a professional stating that they were healthy enough to attend daycare. Risk level: Medium. Corrected by 2024-09-06.
  • Critical Child/Caregiver Ratio - Licensed Child Care Home
Inspector's note: During this inspection, the primary caregiver was the only caregiver present as her husband was not there. There were 2 infants and 8 children ages 18 months to 3yo, for a total of 10 children. The caregiver corrected this by having 3 children between the ages of 18mo-3yo be picked up by parents. She then came into compliance with 2 infants and 5 children ages 18mo-3yo. Risk level: Medium High. Corrected by 2024-08-09.
  • Non-critical Documentation of Training Requirements-Original Training Certification Card or Letter in each Employee's Personnel record
Inspector's note: The primary caregiver could not locate for herself or her husband (who is also a caregiver) their training for the last completed year. The caregiver stated that they should be able to locate it soon but if not, they will recomplete it and send to licensing. Risk level: Medium Low. Corrected by 2024-09-06.
  • Critical AP Initial background checks submitted - At the time a non-client resident 14 or older moves into your home or operation
Inspector's note: The primary caregiver failed to notify licensing and failed to have a background check run prior to her adult son moving back into their home. Risk level: High. Corrected by 2024-08-09.
  • Critical Infant May Not Sleep in a Restrictive Device
Inspector's note: A four month old infant was found to be sleeping in a restrictive device that was placed on top of a king-size bed. There was also a blanket that had been placed on top of the child's stomach and legs. This was corrected at inspection when the caregiver moved the child to a pack-n-play without any other items inside of it. Risk level: High. Corrected by 2024-08-09.
  • Critical Safety - Areas Free From Hazards
Inspector's note: There were multiple safety concerns found that posed a risk to child safety during this visit. For example: there was a bottle of 500mg Acetaminophen on a nightstand in a bedroom that the children were found sleeping in; there was a bottle of Downey Wrinkle Releaser spray and an aerosol can of Degree deodorant in bedrooms children were found sleeping in; there was an open bag of tools found on the ground in a bedroom children were found sleeping in, and there were multiple outlet covers found missing throughout the home. These were corrected at inspection when the children were removed from the rooms with the safety threats and outlet covers were replaced. Risk level: Medium High. Corrected by 2024-08-09.
