Through interviews with staff, it was determined that the cover on a hose connection box was broken on the playground. Inspector's note: During the observation inspection at the facility two infants were observed asleep in restrictive devices and one was asleep on a boppy in a sleep sacks. The infants were not removed immediately from the devices and placed in their cribs but were said to had fallen asleep after they were fed. The babies were eventually removed from the devices or awaken after approximately 3 minutes after the caregivers were instructed to do so. Risk level: High. Corrected by 2025-11-04.
