Inspector's note: Based on information obtained it was found that the building at the operation has not been maintained to be free from hazards. On the playground an area of the surfacing has been ripped open, with parts of the surfacing being raised and creating a tripping hazard. In the same area where the surfacing is ripped, there is a large dip in the ground that is hard to see unless you are on it. This creates a tripping hazard for the children. An area in the infant classroom has laminate tile that has raised and broken edges. Risk level: Medium High. Corrected by 2024-07-31.

Risk level: Medium High. Corrected by 2023-03-24. - Critical Responsibilities of Employees and Caregivers -Demonstrate Competency, Good Judgment, Self-control
Inspector's note: A caregiver showed poor judgement when sharing information of an incident occurring at the center with someone outside the center, instead of with the director.
