Inspector's note: It was determined that at least two children were unsupervised in an empty classroom pushing on a door. While this was occurring, a third child is seen pulling on the door and staff were not aware of what was happening. Risk level: High. Inspector's note: It was determined that the Dutch door in between the classrooms had a broken latch that prevented both halves of the door to be latched. The director did relocate the children to another area of the facility during the inspection and the children will no longer be present on the second level until the fire marshal gives the approval.
