Inspector's note: It was determined that the therapy dog present at the operation bit a child on the side of their face which caused an unsafe situation. The dog's handlers allowed the children to surround the dog instead of approaching the dog two at a time. Risk level: Medium High. Inspector's note: It was determined that the director of the operation did not use good judgement. After the dog had bitten the victim the dog handlers attempted leave for the day. The director talked to the dog handlers and asked them to stay, which they agreed. The director then instructed the handlers to take the therapy dog to the remaining three classrooms where children were present. Risk level: High. Corrected by 2026-07-17.
