- Critical Parental Communication - Situation that Placed a Child at Risk
Inspector's note: Information obtained for this investigation, revealed two infant caregivers were aware an infant was given the wrong breastmilk bottle and the parent was not informed of this mistake. Staff informed the parent of not being able to recognize which bottle belonged to the two infants with the same bottle, in which both had breastmilk. Risk level: High. Corrected by 2024-08-06. - Critical Report Situation Placing Children at Risk
Inspector's note: Based on the information collected for this investigation, it was learned that the operation did not notify licensing when the staff failed to feed an infant their breast milk bottle on two separate occasions. It was discovered that one of the incidents, due to the lack of proper labeling of bottles, uncovered the infant caregiver served an infant the wrong breast milk bottle. Both caregivers in the classroom were aware of the situation and did not informed the director. Therefore, the operation did not notify licensing. Risk level: High. Corrected by 2024-08-06. - Non-critical Written Feeding Instructions - Signed and Dated by Parent or Health-Care Professional
Inspector's note: Information obtained during investigation; determined the same infant was not served their last PM bottle on two separate occasions. Risk level: Medium. Corrected by 2024-08-06.
