Inspector's note: Based on information gathered through the course of the investigation, the operation failed to ensure that a child in care was supervised as required. On the date of the incident, the child was checked into care; however, no caregivers were supervising him at the time he fell into the rock wall and sustained an injury requiring medical attention.
