Inspector's note: The operation combined two groups at the end of nap time to faciltiate a restroom break for a caregiver, but the youngest child was a 17 month old and ended up being mixed with children up to 5 years old. The presence of the infant also left the entire combined group out of ratio. Risk level: Medium. Corrected by 2026-07-08.
