Inspector's note: Based on interviews and videos, two staff did not make a name to face count or a thorough sweep of the playground when transitioning the children from outside to the inside of the building resulting in a child being left on the toddler playground for 1 minute and 3 seconds. The PreK and the afterschool areas were being utilized at this time, but staff were unaware of the child that was left until the child was noticed by a caregiver. Risk level: High. Inspector's note: The plastic containers in two rooms that hold toys were damaged, leaving rough edges that could pose a risk to children. This was corrected when the containers were removed. The bathroom doorknobs in the afterschool room were broken and sharp to the touch. This was accessible to children while opening the bathroom doors. This was temporarily corrected by covering the knobs with tape until the replacement knobs arrive. Risk level: Medium High. Corrected during the inspection.
