Risk level: High. Corrected by 2025-11-21. - Critical Supervision ensures each child?s safety, well-being, including physical proximity and auditory or visual awareness of each child?s on going activity

- Non-critical Written Operational Policies - Transportation
Inspector's note: The operation has a strong written transportation emergency preparedness plan which includes procedures for restroom emergencies while in transport. However, the policies were not followed as there were no planned restroom stops for a field trip that was about an hour and a half drive away. The policies also were not followed when a child urgently had to go to the bathroom. While information gathered in the investigation indicated staff could not pull over at the time due to stand still traffic, they also did not follow their policies by pulling over when they safely could as the child ended up going on the bus as they were unable to hold it. While the parents were notified of the incident and all parents notified of arrival at the center being delayed from the field trip due to traffic, the director was not notified immediately of the restroom emergency by staff which is in their policies. Risk level: Medium. Corrected by 2025-09-11.
- Non-critical Parental Notification of Less Serious Injuries
Inspector's note: Information from the DFPS investigation indicated a parent was not notified until the next day about an injury in care that occurred. Risk level: Medium. Corrected by 2024-12-18.

- Critical Supervision ensures each child?s safety, well-being, including physical proximity and auditory or visual awareness of each child?s on going activity
Inspector's note: Based on the information gathered in a DFPS abuse/neglect investigation, supervision was not in compliance when a child was alone unsupervised. The caregiver did not know the child went back inside the building from the playground and exited, entered, and exited the building and then exited the playground and crossed the street where they were found. Risk level: High. Corrected by 2024-05-30. - Critical Sign-in and Sign-out Tracking System
Inspector's note: Based on information obtained in the DFPS investigation, the sign in and out log was not being updated regularly/was not accurate on the day the incident being investigated occurred. Risk level: Medium High. Corrected by 2024-05-30.
