    El personal no se lavaba las manos cuando debía, o un lavamanos estaba bloqueado, sin jabón ni toallas de papel, o se usaba para otra cosa. Las manos son una de las formas más comunes en que los gérmenes llegan a la comida.
    Redacción oficial (en inglés): Employee observed handling raw chicken with gloved hands then removed gloves and washed hands with only cold water at dish washing station. Employees are required to properly wash hands at designated handwashing stations only, using warm water, liquid hand soap and dry paper towels prior to donning new gloves or whenever contamination occurs. Instructed employee to properly wash hands.

    Los alimentos no se mantenían lo suficientemente fríos o calientes, no se enfriaban con la rapidez necesaria, o no se cocinaban o recalentaban a la temperatura correcta. Las bacterias crecen rápido en la "zona de peligro" entre 41 °F y 135 °F (5 °C a 57 °C), por lo que esta es una de las revisiones de seguridad alimentaria más importantes.

Employee stated the breading ingredients are replaced every 2 weeks. Potentially hazardous foods (PHFs) such as ingredients used for breading raw oysters are required to be maintained under time or temperature control. Maintain breading ingredients at 41F or utilize time as a public health control (TPHC). Use of TPHC requires date/time stamps and written procedures. Breading ingredients voluntarily discarded.
