Observed Vermin: Documented in the following areas: - Observed live cockroaches of different life stages in the back storage area in the following areas: --- One in the stack of pots and pans underneath the central storage area --- One around the packaged food containers in a cardboard box in the central storage area --- One on top of the laundry machine in the side storage area - Observed numerous flies and gnats throughout the facility, especially in the back storage area. - Observed old rodent droppings throughout the back storage area, especially in the central storage area and on the floor. Observed old droppings on top of beverage boxes. 2. Photographs: Taken for documentation purposes. 3. Supervisor Notified: Suzanne Lew 4. Notification: The person in charge during inspection, Tina Nguyen, has been informed that the facility must close immediately. [CA]: The premises of each food facility must be maintained free of vermin. A facility cannot operate if there is a vermin infestation that leads to contamination of food contact surfaces, packaging, utensils, food equipment, or adulteration of food. The facility is required to cease operations immediately and must remain closed until all corrective actions on the provided checklist are completed.
