A permit shall be issued by this department when investigation has determined that the proposed facility and its method of operation meets the specifications of the approved plans or conforms to the requirements set forth by this department.
To reopen facility must:
1) Submit the business license of the current owner and pay delinquent annual permit fee.
OR
Submit a new permit application and facility evaluation request form and go through change of ownership permitting process.
AND
2)Obtain approval for water heater from this Agency's plan check department.

2)Per employee he only keeps eggs for 4 hours and then discards them. Employee labeled flat of eggs for 10AM-2:00PM. Facility shall provide SOPs for time as a public health control. Operator will be emailed TPHC paperwork.
