Observed on live juvenile cockroach on the wall by the front entrance. Observed one dead cockroach on lower shelf of preparation table, on the floor to the door by the dry storage. 2. Photographs: Taken for documentation purposes. 3. Supervisor Notified: S. Lew 4. Notification: The person in charge during inspection, John, has been informed that the facility must remain closed immediately.
