    Nota del inspector (en inglés): 4-501.114 Maintain sanitizer at correct concentrations when being used to sanitize. -P -Observation: dishes were being washed in hot temp dishwasher and water was not reaching 160F. 3 comp sink with QUAT sanitizer was reading 0ppm. -Corrected During Inspection (CDI): after several attempts; water reached 160F. PIC also fix the bucket sanitizer and concentration went to 200ppm. Environmental Health Specialist (EHS) asked employee to rewash dishes and food employee voluntarily did it. REPEAT 4-601.11 (A) Equipment; Food-Contact Surfaces; Nonfood Contact Surfaces; and Utensils (Pf) -Observation: dishes and containers in clean area had dry food sticked on it and accumulation in addition to sticky residue. -Person in Charge (PIC) voluntarily placed dishes in dirty dishes to be washed and sticker removed.

Onion soup was being cool down in a deep container with tight fitting lid and it was not cooled down to 41F within 6h. -CDI: PIC voluntarily open container for proper cooling the lettuce and discarded the onion soup.

The thermometer must be readily accessible. -Pf -Observation: thermometer for hot temp dishwasher is no longer working and facility doesn't have a way to measure water temperature in dishwasher. -PIC will have until May 23; 2026 to have one available at the facility. A verification will be done by the time mentioned; in case establishment is able to fix this violation prior to the due date; make sure to get in contact with EHS using the cellphone printed in this inspection.

Ice bin in soda machine had severe black accumulation that is possible to see when lid is completely open; between lid and ice. -Reach-in freezer had severe food debris accumulation in the bottom inside. -Make sure to clean as often as necessary to avoid pink accumulation. -EHS discussed the matter with PIC.

    Nota del inspector (en inglés): 2-301.14 (I); Priority; Cook observed handling raw chicken with glove; taking glove off; and replacing with new glove to begin working with ready to eat foods and handle clean utensils without first washing hands. Wash hands after engaging in other activities that contaminate hands. CDI- EHS educated the cook on handwashing. Cook washed his hands. PIC states that is not policy and will re-train staff. No points as this is the first inspection.

Retain tag on the bag of shellstock until empty. Record date on the tag when the last shellstock from the container is used. Store tags in chronological order for 90 days. CDI- EHS educated PIC on the importance of shell stock and record keeping. PIC will ensure all future shellstock tags will be recorded and kept properly.

Observed cooked steak without date mark. Observed noodles made yesterday without date mark. Gnocchi made 10/26 without date mark. Discard the food requiring date labels once time/temperature window has expired; if it is not been labeled; or if the label is incorrect. Date mark/label all TCS foods that are ready-to-eat once opened or prepared and held more than 24hrs. CDI- PIC voluntarily discarded out of date product and confrimed dates for other products that were not labeled.

Resurface or replace cutting surfaces that can no longer be effectively cleaned and sanitized.
