On grill; pan of cooked onions in pan of water holding at 107F. Under heat lamp; tater tots holding at 114F. When asked; PIC stated foods normally not out more than 4 hours; but no actual TPHC in place. Maintain hot Temperature Control for Safety (TCS) foods at 135F and above. Verify foods reach the proper final cook or reheat temperatures before placing in hot-holding. PIC stated the roast beef was recently sliced. Maintain cold Temperature Control for Safety (TCS) foods at 41F and less. CDI- TCS foods exceeding 41F voluntarily discarded by PIC during inspection; except recently prepped roast beef moved to walk-in for fast-chill. Recommend using metal pans for TCS foods in top section of prep units; and individual lids for pans of chunky foods (ex.-hard boiled eggs; large chunks of meat; stacks of cheese). Recommend keeping TCS foods below the fill line notch of the pans when in the top section of prep units. TIME/TEMPERATURE CONTROL FOR SAFETY (TCS) foods shall be thawed: Under refrigeration that maintains the FOOD temperature at 5C (41F) or less; or Completely submerged under running water; At a water temperature of 21C (70F) or below; With sufficient water velocity to agitate and float off loose particles in an overflow; and For a period of time that does not allow thawed portions of a raw animal FOOD requiring cooking to be above 5C (41F); for more than 4 hours including The time the FOOD is exposed to the running water and the time needed for preparation for cooking. Long prep table and short side table with rusted lower shelves. EQUIPMENT shall be maintained in a state of good repair and condition. EQUIPMENT components such as doors; seals; hinges; fasteners; and kick plates shall be kept intact; tight; and adjusted in accordance with manufacturers specifications. Replace the rusty racks/carts. Floor drain cover sunk sideways down into the drain right at the main prep line creating a trip hazard. PHYSICAL FACILITIES shall be maintained in good repair. Repair the flooring so smooth and cleanable. [REPEAT] 6-201.11; Core; Drink machine syrup boxes room has unfinished drywall walls. Wall behind the smallwares storage rack has wall damage with some of the wall surface gone. Floors; floor coverings; walls; wall coverings; and ceilings shall be designed; constructed; and installed so they are SMOOTH and EASILY CLEANABLE. Maintain floor; walls and ceiling in good repair. Repair the wall damage. Install RFP board or other approved restaurant wall surface in the soda syrups room. [REPEAT]
