Walk-in cooler 2 has damaged/leaking ceiling; wall and floor surfaces with large amount of water accumulation on floor. Ice was accumulated on exterior bottom of evaporator. Physical facilities shall be kept in good repair; with floors and walls smooth and cleanable. Repair damaged surfaces surfaces/fixtures and leaks stated above. Replace burned out/improperly functioning light fixtures. 6-501.114; Core - Observed many pieces of equipment in facility that are nonfunctional or broken. The premises shall be free of items that are unnecessary to the operation or maintenance of the establishment such as equipment that is nonfunctional or no longer used. Remove unnecessary/nonfunctional equipment from facility or repair equipment that needs repair.

CDI- PIC transferred cantaloupe/melons to the walk-in to rapidly cool. Boiled eggs were voluntarily discarded.
