SMALL FLY ACTIVITY WAS NOTED NEAR THE UTILITY SINK AND WALLS IN THE BACK STORAGE AREA, THE WALLS IN THE DINING AREA, AND AROUND THE WINDOWS. OBSERVED A SIGNIFICANT NUMBER OF DEAD FLIES UNDER THE WINDOW IN THE DINING AREA AND THE BACK STORAGE AREA. OBSERVED EVIDENCE OF RODENT ACTIVITY ON THE PREMISES. OBSERVED APPROXIMATELY 40 MOUSE DROPPINGS ON THE FLOOR OF THE REAR STORAGE AREA BEHIND THE CHEST FREEZER, UNDER THE STORAGE SHELVES, AND ALONG THE WALLS THROUGHOUT PREMISSES. OBSERVED APRX. 15 LIVE ANTS ARE COMING UNDER THE METAL THRESHOLD BETWEEN THE SERVING AREA AND THE KITCHEN. MANAGEMENT INSTRUCTED TO CLEAN AND REMOVE ALL RODENT DROPPINGS AND DEAD FLIES FROM ALL AFFECTED AREAS. ADDITIONAL PEST CONTROL SERVICE IS NEEDED TO ELIMINATE THE PEST ACTIVITY. PRIORITY FOUNDATION #7-38-020(A). NO CITATION ISSUED.
