OBSERVED NO CITY OF CHICAGO CERTIFIED FOOD MANAGER WITH THE ORIGINAL CITY OF CHICAGO FOOD SERVICE MANAGER CERTIFICATE, ON SITE DURING THE INSPECTION. ONLY A CITY OF CHICAGO FOOD SERVICE MANAGER CERTIFICATE WAS OBSERVED, BUT THE PERSON WHO THE CERTIFICATE BELONGS TO WAS NOT ON SITE. INSTRUCTED A CITY OF CHICAGO CERTIFIED FOOD MANAGER MUST BE ON SITE AT ALL TIMES. PRIORITY FOUNDATION VIOLATION 7-38-012, NO CITATION ISSUED

OBSERVED SPIDER WEBS AND DEAD BUGS ON THE FLOOR BEHIND THE DOOR OF THE FOOD PREP AREA, ON THE FLOORS AND WINDOWSILLS IN MULTIPLE CLASSROOMS, AND IN THE LOWER COMPARTMENT OF THE CUBBY UNITS. INSTRUCTED TO REMOVE DEAD BUGS AND SPIDER WEBS, SANITIZE AND MAINTAIN.
