OBSERVED APPROX. 15 LIVE SMALL FLYING INSECTS ON THE WALLS, CEILING TILES, BOXES OF STOCK AND DRY STORAGE SHELVES IN THE FOOD PREP AREA, SERVICE LINE AREA, INSIDE AND OUTSIDE OF THE DRY STORAGE ROOM AND WASHROOM AND AROUND THE MOP SINK. ADDITIONAL PEST CONTROL SERVICE IS NEEDED TO ELIMINATE THE PEST ACTIVITY. PRIORITY FOUNDATION VIOLATION 7-38-020(A).
