OBSERVED FACILITY WITH NO BODILY FLUID SPILL KIT AND NO CORRESPONDING CLEAN UP PROCEDURE. INSTRUCTED FACILITY TO MAINTAIN A COMPLETE BODILY FLUID SPILL KIT AND WRITTEN CLEAN-UP PROCEDURE ON SITE AT ALL TIMES. PRIORITY FOUNDATION VIOLATION #7-38-005. CITATION ISSUED.

OBSERVED EVIDENCE OF PEST ACTIVITY THROUGHOUT THE FACILITY. NOTED APPROX. 20 SCATTERED MOUSE DROPPINGS ON CARDBOARD CONTAINERS WHERE CHIPS ARE STORED AND APPROX. 10 RAT DROPPINGS IN REAR ON FLOOR NEAR RAMP. MANAGEMENT INSTRUCTED TO REMOVE ALL RODENT DROPPINGS FROM ALL AFFECTED AREAS, CLEAN, AND SANITIZE. ADDITIONAL PEST CONTROL SERVICE IS NEEDED TO ELIMINATE THE PEST ACTIVITY. PRIORITY FOUNDATION #7-38-020(A). CITATION ISSUED.
