The name of the location is "City Table" I discussed with the PIC to provide an updated application for the DBA Change from "Juniper & Ivy" A fillable application will be Emailed to the PIC when filled out return to ISDHealth@Boston.gov PIC also explaining that the location will be closing in December for renovations when completed the location will re-open under the new DBA of "Juniper & Ivy" It was also discussed with the PIC to call the ISD Health office to schedule a plan review.(617-635-5326)
