Use a consumer advisory to inform customers of the increased risk of consuming raw or undercooked animal foods.
Establishment allows consumers to order eggs to be served as "over-easy", but there's no asterisk or consumer advisory on the menu to inform consumers the risks associated with eating raw or undercooked eggs. Informed the chef and manager on duty (MOD) to ensure they update their menus (app, screens, and online) by adding the required consumer advisory notice. Provided MOD copy of MDH's Consumer Advisory factsheet with this report.
Food was not kept cold enough, hot enough, cooled fast enough, or cooked or reheated to the right temperature. Bacteria grow quickly in the "danger zone" between 41°F and 135°F, which makes this one of the most important food-safety checks.

Establishment sells prepacked sushi from Sushi Ave and has a consumer advisory posted by the sushi display case. However, the advisory was missing the disclosure part.
