Keep clean utensils that are unwrapped in trays and holders with their handles up.
Most of the silverware was stored in containers with the handles up but some extra silverware was stored in a tray jumbled so that the handles and eating surfaces were mixed with the potential that someone grabbing a handle could also touch the eating surfaces of other utensils. This was immediately corrected before the kids arrived.
Food was stored or handled in a way that could let germs or other material get into it, such as raw meat stored above ready-to-eat food or uncovered food. Cross-contamination is a common cause of foodborne illness.

The milk dispensing tubes were cut horizontally, allowing for milk to stay caught in the tube and left overnight grow bacteria. This was immediately cut at an angle, allowing for the non refrigerated milk in the tube to drain out.
