4-501.11; Core; Service Request (28554973) from last routine inspection regarding the ice accumulation in the walk-in freezer was repaired on 5/15/2026. Technician stated "Repaired refrigerant leak. Cleaned condenser coil. Made defrost adjustments". Today; ice was found accumulating on the evaporator coil in the walk-in freezer again. Sheet trays are currently placed under unit to catch the drip. Equipment and components shall be maintained in a state of good repair and condition. Since May; PIC has submitted two other service requests because the walk-in freezer unit was consistently holding around 20F. Submit another request to repair the leaking evaporator unit. *Full points not deducted due to repairs being completed between last routine inspection and today.

Items that are unnecessary to the operation or maintenance of the establishment such as equipment that is nonfunctional or no longer used shall be removed from the premises. Remove or repair nonfunctional equipment. Points are now being assessed for repeat violation.
