3-304.14; Core; Sanitizer trays observed with a concentration of 0ppm quat and solutions were cloudy. Cloths in-use for wiping counters and other equipment surfaces shall be held between uses in a chemical sanitizing solution at the proper concentration (200-400ppm quat). CDI- Buckets were replenished with sanitizer at the correct concentration.

4-501.11;Core; Tall reach-in cooler in BOH has an ambient air temperature of 48F. Temperature in this unit was manually logged this morning around 5am and temped at 38F. The only TCS food found in this unit was sweet cream which was prepared this morning a few hours ago. Facility also has a Bluetooth thermometer in this unit but was not showing data when verified on tablet. Sweet cream was relocated to alternate refrigeration. Equipment shall be maintained in a state of good repair and proper adjustment. Since this unit stopped functioning properly this morning; PIC submitted a service request. PIC agreed to only use this unit for non-TCS beverages until unit is repaired and capable of holding at 41F or below.

4-903.11(A); (B) and (D); Core; A few plastic food containers above the 3-compartment sink were found stacked while still wet. Observed a few clean utensils hanging off of shelving mounted above the wash basin of the 3-compartment sink where splash contamination may occur. Cleaned equipment and utensils shall be stored in a self draining position that allows air drying and shall be stored where they are not exposed to splash; dust; or other contamination. Ensure these containers are completely dry before stacking together and relocate utensils that are hanging off of the shelving that is mounted above the wash basin of the 3-compartment sink.
