Mobile FSO/RFE is using an unapproved method for disposing of liquid wastes. Person in Charge stated that mop water is being dumped in the back alley. Liquid waste cannot be dumped this way. The Person in charge stated that moving forward, mop water will be disposed of through the toilet connected to the sanitary sewer system.

Food-contact surfaces were not clean to the sight and touch. Observed biofilm buildup inside soda gun nozzles. Discussed with the PIC that, in addition to soaking in a cleaning solution, the nozzles must also be mechanically scrubbed to ensure proper removal of buildup.

Food employees and conditional employees are not informed in a verifiable manner of their responsibilities to report to the person in charge, information about their health as it relates to diseases transmissible through food. Observed that the facility does not have an employee health reporting policy in place. The Person in Charge was provided with the Cincinnati Health Department employee health reporting form and instructed to have all food employees review, acknowledge, and sign the document.
