A food label did not include the name of the food source of each major food allergen contained in a food unless the food source is already part of the common or usual name of the respective ingredient. Observed self service food items of salads and other TCS food did not have proper labels with major allergen information and ingredients present.
Food came from an unapproved source, was missing required date marks or labels, or was past its use-by date. Knowing where food came from and how old it is helps prevent illness.

Nonfood-contact surfaces are not cleaned frequently enough. Mold on shelves and sleeves where chocolate milk stored in the cooler.
Dishes, equipment, or surfaces that touch food were not properly washed, rinsed, and sanitized, or sanitizer was not at the right strength. Clean surfaces help stop germs from spreading to food.
