Single-service and single-use articles were not stored in the original protective package or were stored by means that did not protect them from contamination. Observed boxed single use articles stored on facility flooring. Discontinue practice and ensure articles are stored >= 6" inches above flooring.

Ready-to-eat food was not protected from cross-contamination by raw animal food. Observed raw shell eggs improperly stored with RTE foods. PIC to ensure eggs are properly stored and separated at bottom of refrigerator.
