4.5(A)(1) - VII. Protection from Contamination - Equipment Food-Contact Surfaces and Utensils: Clean to Sight and Touch
Food-contact surfaces were not clean to the sight and touch. Observed biofilm growth inside one of the ice machines. To ensure proper sanitation, all ice must be discarded, and the interior surfaces thoroughly cleaned and sanitized before returning the machine to uses.
Food was stored or handled in a way that could let germs or other material get into it, such as raw meat stored above ready-to-eat food or uncovered food. Cross-contamination is a common cause of foodborne illness.

Temperature measuring devices are missing, not correctly placed, not readable, or of improper design. Observed that the dairy cooler is missing a thermometer. A temperature measuring device must be present to ensure proper monitoring.
Food was not kept cold enough, hot enough, cooled fast enough, or cooked or reheated to the right temperature. Bacteria grow quickly in the "danger zone" between 41°F and 135°F, which makes this one of the most important food-safety checks.
More about food kept at an unsafe temperature3.2(K) - In-use utensils - between-use storage
In-use utensils are improperly stored. Observed ice scoops stored above boxes, which can lead to contamination. Ice dispensing utensils must be stored in a clean, designated container outside the ice machine to prevent contamination and ensure food safety.
Equipment, utensils, thermometers, cutting boards, or single-use items were not in good condition, not the right type, or not stored properly.
