Shield covers are missing at the lights around the kitchen area. [CA] Light bulbs shall be shielded, coated or otherwise shatter-resistant in areas where there is unpackaged foods, clean equipment, utensils, linens, or unwrapped single use articles.
Floors, walls, ceilings, lighting, ventilation, or garbage areas needed repair or cleaning. These are usually less urgent than food-handling problems but still need to be fixed.

1. Observed Vermin: Documented in the following areas: - Multiple live cockroaches from different life cycles noted around the 3-Compartment sink - Multiple noted within the sticky traps around the kitchen area. - One food within the freezer chest of the facility. 2. Photographs: Taken for documentation purposes. 3. Supervisor Notified: Samantha K. 4. Notification: The person in charge during inspection, Bunlong Hok , has been informed that the facility must close immediately. [CA]: The premises of each food facility must be maintained free of vermin. A facility cannot operate if there is a vermin infestation that leads to contamination of food contact surfaces, packaging, utensils, food equipment, or adulteration of food. The facility is required to cease operations immediately and must remain closed until all corrective actions on the provided checklist are completed. Requirements Before Reopening: 1. Email the signed and completed Reopening Checklist to the assigned inspector. 2. Submit a copy of the pest control report from a licensed provider.
The inspector found mice, rats, cockroaches, flies, or evidence of them (like droppings) inside the business, or conditions that could attract them. Pests can spread bacteria to food and surfaces, so this is treated as one of the more serious findings.

1. Large gaps noted behind the FRP around the 3-Compartment sink. [CA] Repair holes in structure and maintain in good repair to prevent pest harborage. Seal FRP back onto the wall properly. 2. Lots of food debris noted around the floor and under equipment at the time of inspection. [CA] Regularly clean under cooking equipment to prevent accumulation of debris. 3. Damaged concrete noted around the grease trap. [CA] Seal large hole around the grease trap properly.
Dishes, equipment, or surfaces that touch food were not properly washed, rinsed, and sanitized, or sanitizer was not at the right strength. Clean surfaces help stop germs from spreading to food.

Only hand wash station within the prep area is blocked at the time of inspection. [CA] Hand washing facility shall be clean, unobstructed, and accessible at all times. [COS] Items around the hand wash station were removed. Hand wash is now free and unobstructed. REPEAT VIOLATION. Failure to comply may result in enforcement action.
Staff were not washing their hands when required, or a handwashing sink was blocked, missing soap or paper towels, or used for something else. Hands are one of the most common ways germs reach food.

Only hand wash station within the facility is blocked with a bucket at the time of inspection. [CA] Hand washing facility shall be clean, unobstructed, and accessible at all times. [COS] Bucket was removed and hand wash is accessible. Discontinue this practice. REPEAT VIOLATION. Failure to comply my result in enforcement action.
