1. In the kitchen area, on the floor behind the undercounter pie oven, a nymph was observed on a fry. 2. In the front of house area, near the frappe and milkshake machine, observed two adult cockroaches and a nymph. 3. In the dining area, on the ceiling above the self-serve soda dispensers an accumulation of cockroach droppings were observed on the ceiling tiles. 4. An accumulation of cockroach droppings were observed on the ceiling tiles above the lockers. Note: Facility receives monthly pest control from EcoLab. Pest control reports dated on 11/27/24 and 12/24/24 both indicate no cockroach activity and treatment with both bait and crack and crevice pesticide. [CA] The premises of each food facility shall be kept free of vermin. A food facility shall not operate when there is a vermin infestation that has resulted in the contamination of food contact surfaces, food packaging, utensils, food equipment, or adulteration of food(s). The food facility shall cease operation of the food facility immediately. The food facility shall remain closed until: there is no longer evidence of a vermin infestation; all contaminated surfaces have been cleaned and sanitized; and contributing factors such as cleaning, repairs, and the elimination of harborages have been resolved.
